Do you give her treats that are good for removing plaque.
NOT rawhide or Greenies or bones- those are SO BAD for dogs.

If you see very little or no improvement after trying these I would say take the dog to the vet. Its teeth could be infected. Sometimes needs to be pulled.

Bad breath usually indicates gum disease from tarter build up. When did your dog have its last dental? sounds like one is due, especially if you feed canned food.
Make sure there is nothing caught along the gum line - top or bottom, or behind the teeth
You can start brushing his teeth - give baby carrots, apple slices, raw shin bones from the butcher, but most importatly get the dog to the vet.
